Filling in your center's forms

The forms your center asks your family for: where they are, filling one in, and what happens after you send it.

For parents and guardians

Where to find them

Go to More and tap Forms, under Your children. It lists the forms your center has asked your family to fill in: an emergency card, a permission form, a questionnaire about your child, and so on. Your center writes its own forms, so what you see depends on your center.

If it says Nothing to fill in, your center hasn't asked for anything. Forms are in the app only; the family portal on the web doesn't have them.

Reading the list

The top card says You're all caught up, or how many forms need you. Each form shows its title and, if your center needs it returned, Your center needs this one back.

Some forms are filled in once for each child and some once for the whole family, so a form can have a line for each child. Each line says who it's for and where it stands:

  • Needs you: not sent yet, with the due date if there is one.
  • Overdue: the due date has passed.
  • Done: sent, with the date.
  • Expiring soon or Expired — fill in again: see Forms that come back each year.

Filling one in

  1. Open it Tap Fill in on the line for the child (or family) it's for.
  2. Answer the questions Questions can ask for a few words or a longer answer, a number, a date, yes or no, or a choice from a list. Some ask you to tick a box to confirm you've read something; forms don't ask for a drawn signature. A question can appear only after a particular answer to an earlier one.
  3. Check what's left The line above the button says how many required questions you've answered. If you missed one, the app marks it.
  4. Send it Tap Submit form. You'll see Form sent.

If a question says it needs a newer version of the app, update RoundUp from the App Store or Google Play, or ask your center for a paper copy.

After you send it

A form you've sent is read-only. View answers shows exactly what you sent, worded as the form was on the day you sent it. There's no edit button.

If something needs correcting, ask your center. They can hand the form back to you, and it's on your list to fill in again.

Each line is filled in once. If another parent or guardian in your family has already sent it, it shows as done for you too.

Forms that come back each year

Your center can say how long a form stays good, such as a year for an emergency card. Thirty days before yours runs out, its line says Expiring soon; on the day, Expired — fill in again. Tap Fill in and send a new one. You start from a blank form, not your old answers.

Reminders

For a form with a due date, RoundUp reminds you three days before it's due and again the day after, if it still isn't in. For a form that runs out, it reminds you 30 days before and on the day. Reminders arrive in Notifications in More, and on your phone, as long as Announcements is switched on in Settings. See Turning notifications on or off.

Who can see your answers

Your center's staff can read what you send. A form never shows you which other families were asked, and other families can't see your answers.

Sending a document instead

If your center wants a copy of a paper document, such as a vaccine record or a custody order, that's Send a document, not Forms. See Sending your center a document.

Still stuck?

Your center's director can see your family's account and can fix most things fastest. If the app itself is misbehaving, that one's ours.
