Sending your center a document

Immunization records, allergy plans, custody orders and other paperwork, sent from your phone.

For parents and guardians

What it's for

When your center needs a copy of something — an updated vaccine card, a doctor's allergy plan, a custody order, a subsidy letter, enrollment paperwork — you can send a photo or PDF of it from the app instead of bringing it in.

Sending a document

  1. Open it Go to More and tap Send a document, under Your children.
  2. Start Tap Upload a document.
  3. Say what it is Choose the Child it's for, then What is it?: Immunization record, Medical / allergy plan, Custody or court order, Subsidy / assistance, Enrollment paperwork or Other. Give it a Name, such as 2026 vaccine card.
  4. Add the file Choose Photo or PDF or file. Photos (JPEG, PNG, HEIC) and PDFs up to 15 MB are accepted.
  5. Send Tap Send. You'll see Sent — your center will review it.

A photo of a paper document works fine. Lay it flat in good light so every word can be read.

After you send it

Everything you've sent is listed on the same screen, with its status:

  • Waiting for review — your center hasn't looked at it yet.
  • Reviewed — staff have checked it.
  • Needs a new copy — something's wrong with it, such as a blurry photo or the wrong page. Your center's note says what; send a new one.

Tap Open to look at something you sent.

Immunization records

For an immunization record, the app may also ask Which vaccine is this proof for?. It's optional. Picking one attaches your upload to that vaccine on your child's health record, so staff see the proof right beside it.

Before your child is enrolled

If your child doesn't have a place yet, there's no app account to send from. A center that needs documents emails you a link to your inquiry page instead. See Sending documents in the article on tours and inquiries.

Who can see it

Only your family and your center's staff can see what you send. It's stored with your child's records, not on your phone.

Custody orders

Send the order here so your center has a copy, but tell the director in person or by phone as well, and say what it means for pickups. A document waiting for review protects nobody at five o'clock. See Authorized pickups.

Still stuck?

Your center's director can see your family's account and can fix most things fastest. If the app itself is misbehaving, that one's ours.
