Immunization records and Oregon reports
Each child's vaccine records and exemptions, the state registry page, and the reports Oregon asks child care facilities for.
A child's records
Immunizations are kept on each child's profile, in the app and in the director portal, one record per vaccine dose. The portal's Records & reports → Compliance page lists children whose immunizations need attention. Oregon's and the immunization registry pages are part of the Health module; if they say Turned off for this center, it's off for yours.
Exemptions
When a child's immunization status is Exempt, their Edit dialog in the portal asks what kind: Medical or Religion or conscience. The child's record shows it under Health. Your state's own rules decide which exemptions count; Washington, for example, no longer accepts a personal exemption for measles, mumps and rubella.
The immunization registry
Records & reports → Immunizations shows whether your state's registry is connected, with a Connection checklist of what's still needed. Once every step is done, Check registry looks a child up, and you review what would be added, what's already on file, what conflicts and what's skipped before choosing Add records. Imported records are marked From registry, and nothing already on file is changed or removed.
Until your state's lookup is connected, you can still Paste a registry response (HL7) copied from the registry's own web tools, for a lookup that found exactly one child. It's read once to build the preview and isn't saved. For Oregon's ALERT IIS, a lookup that finds no one and one that finds too many children give the same answer, so the page can't tell which it was.
Oregon immunization reports
For centers in Oregon, Records & reports → Oregon reports runs Oregon's immunization reports, assessed by Oregon's school and child care rules:
- The Immunization Report's Section A, Sections B and D, and Section E.
- The Student Immunization Report, filtered by grade and status, in five sort orders.
- A Susceptibility list for one vaccine.
A child's Oregon record
The reports read each child's Oregon immunization record, which mirrors their Certificate of Immunization Status (CIS). Open it with Open Oregon record.
- Fill from vaccine records starts it from the dates on the child's profile, earliest first. Nothing is saved until you choose Save record.
- Copy the CIS exactly as written, dose by dose. Don't reorder dates: Oregon shows a date out of order as insufficient.
- Record nonmedical and medical exemptions, immunity documentation (such as a history of chickenpox), the grade, and the Section D follow-up dates.
- Tick The CIS is signed by the parent or guardian. An unsigned record is assessed as insufficient unless it holds a medical exemption. A record printed from ALERT IIS counts as signed: tick This record is printed from ALERT IIS.
The page shows the assessment of each vaccine and of the record as a whole.
Running a report
Choose the report and, if you need them, an Assessment date and Enrollment cutoff; left blank, they're today. Children enrolled after the cutoff are left out, and withdrawn children never appear. Reports print in landscape with the system, facility and dates Oregon asks for.
Each report lists the children it can't assess yet: those with no date of birth, and those with vaccine dates on their profile but no Oregon record, who count as having no record until you fill one in. Fill those in before you send anything to the state.
Ontario and Alberta
Ontario and Alberta centres pick from their province's vaccine names, including combined shots such as DTaP-IPV-Hib. For an Ontario centre, an exemption shows the approved form: 010-3041 (Statement of Medical Exemption) or 010-3042 (Statement of Conscience or Religious Belief), and the compliance status lists children not yet in school who have neither a record nor an approved exemption (O. Reg. 137/15, s. 35). See Ontario, Alberta and B.C. compliance.
